Memory Care Connections (MCC)

MCC is a multi-faceted prevention, intervention and support program for Door County residents experiencing memory loss and their caregivers.

Organization:

Family Centers of Door County

The mission of Family Centers of Door County is to strengthen and support families and communities. Located at five sites throughout the county, the Centers offer a wide variety of social, supportive and educational programs for people of all ages, with a goal of strengthening family life and thereby preventing family violence.

Brussels, WI

How does it work?

MCC offers to families dealing with dementia a variety of services including care planning, education. The program collaborates with other community organizations to improve outreach and service delivery.

Why is it successful?

The program has increased awareness and identification of dementia among the residents of the county it serves, has improved access to available medical and social services and facilitates coordination between the two.
