IL - Advanced Directive Information (2011)

This bill would require the Department of Public Health to publish the Department of Public Health Uniform DNR Advance Directive form in Spanish. In addition, the form must also meet the minimum national requirements to be considered a Physician Orders for Life-Sustaining Treatment (POLST) form.
