Personalize Your Care Act (2011)

This bill would allow for reimbursement through Medicaid and Medicare for a voluntary advance care planning consultation with a physician, nurse practitioner, or physician assistant. The bill would also direct the Secretary of HHS to provide grants for states to develop POLST (Physician's Order for Life Sustaining Treatment) programs; create a standard for advanced care directives in electronic health records, and would make an advanced directive effective outside the state in which it was created.
