CA - Long-term Care Services (2011)This bill would require hospitals to provide discharge information to patients both orally and in writing if it is anticipated that the patient will need additional care after discharge. This bill would also require the State Dept. of Health Care Services to develop or identify an assessment tool by July 1, 2013, to assist eligible consumers to find long-term care services. The bill would require counties to develop long-term care management plans for specified persons and require the county to assign a care manager for each long-term care facility in the county. Long-term care facilities would have to clearly post the contact information for the designated care manager in their county. Note: Bill was introduced 12/6/2010.
