Medicare Care Transitions Program Act (2009)Federal bill would establish a Care Transitions Program within Medicare in which outcomes-based payments are made to community-based suppliers for providing care transition services to an individual after discharge, for improving quality of care, and for reducing rehospitalization rates. These transition services include self-management goal setting, medication self-management support, help arranging follow up care, help maintaining personal health record, and help preparing for future medical appointments.
