Caregiver Eligibility: 

Anyone caring for someone who meets care receiver eligibility criteria

Care Recipient Eligibility: 

Multiple requirements:
- Adults with a diagnosis of Alzheimer's disease or related dementia
- Low income
- Not enrolled in Connecticut Homecare Program
- Able to afford co-payment for service
