Commcare Waiver
Government program assists adults with Traumatic Brain Injury who require nursing level care (as provided by a special rehabilitation facility*) to remain in the community by providing home and community based services, including:
- Care management
- In-home care
- Respite
- Adult day care
- Home modifications
- Transportation
- Counseling
- Vocational rehabilitation
* In the state of PA, a nursing facility is a special rehabilitation facility (SRF) if more than 70% of the residents have a neurological/neuromuscular diagnosis and severe functional limitations

Multiple requirements:
- Adults age 21 and older
- Low income (Medicaid eligible)
- Have a diagnosis of TBI
- Experience substantial limitation in performing activities of everyday life
- Require nursing level of care (as provided by a special rehabilitation facility*)
