PACE programs in Colorado coordinate and provide the entire continuum of care and services to their older enrollees with chronic care needs to enable them to continue living safely in the community as long as possible. Care and services include:

Colorado Co-Op for Hospice and Palliative Care (COCHPC)

Provides information about end-of-life care. Its website includes a search tool which allows users to locate hospice and palliative care programs for individuals who have advanced illness or have been considered terminally ill by a physician.

Colorado Medicaid Nurse Advice Line

The Colorado Medicaid Nurse Advice Line provides Medicaid members free around-the-clock access to medical information and advice.

University of Colorado Cancer Center

Offers comprehensive medical care (diagnosis, treatment, and follow-up) to people living with cancer. A helpline is also available for anyone affected by cancer including patients and family members to speak with trained counselors who provide emotional support and referrals to community resources.

The Colorado Health Care Association

Offers online information on long term care and provides lists of nursing homes and assisted living facilities.

The Colorado Department of Public Health and Environment

Government agency responsible for health issues in the state of Colorado including inspecting and licensing long-term care providers. Provides online directories of:
- Home health care agencies
- Nursing homes
- Assisted living facilities
- Hospice
- Adult day care facilities

Senior Health Insurance Assistance Program

Assists individuals and families with eligibility questions and problems related to Medicare, including prescription drug coverage under Medicare Part D, managed care, long-term care planning, health insurance and other health care issues.

Rocky Mountain Multiple Sclerosis (MS) Center

Offers comprehensive medical care (diagnosis, treatment, and follow-up) to people living with MS. Other services include:
- Counseling
- Adult day care program
- Hydrotherapy
- Fatigue management
- Rehabilitation clinic
An information and referral service is available to anyone affected by MS including patients and family members.

Rocky Mountain Human Services

Non-profit organization assists people with disabilities remain independent in their communities by offering the following services:
- In-home care
- Transportation
- Assessment
- Care management
- Counseling
- Respite
Also manages the Traumatic Brain Injury (TBI) Trust Fund program that provides financial assistance to people with TBI for the purchase of home and community based services.

Rocky Mountain Cancer Centers

Provides online listings of national and local support organizations and programs for people with a diagnosis of cancer and their families.
