North Carolina Division of Medical Assistance

Assists Medicaid eligible North Carolinians who live in inpatient facilities to move into their own homes and communities with supports.

North Carolina Association of Long Term Care Facilities

Statewide association for providers of Assisted Living for seniors. Provides directory of participating assisted living facilities by county and/or city.

North Carolina Area Agencies on Aging

AAAs offer aging services, including:
- Information and Assistance
- Care Management
- Nutrition Services (i.e., Meals on Wheels)
- Legal Assistance
- Services for family caregivers through the Family Caregiver Support Program (FCSP)
State Division for Aging Services website links to 4 AAAs and also provides information about all state aging and caregiver programs.

Home and Hospice Care of North Carolina

Represents over 600 licensed agencies across the State, providing health and health-related services in the home and community. Provides information and assistance in locating a hospice in Care recipient's area.

Home and Community Care Block Grant Services

Government program that provides home and community-based services for older adults and their unpaid caregivers. Services include:
- In-home care
- Meals (Congregate/delivered)
- Adult day services
- Care management
- Transportation and more

Family Caregiver Support Program (FCSP)

FCSP services for family caregivers are provided through local area agencies on aging (AAAs) and include:
- Information
- Assistance
- Care management
- Individual counseling, support groups and caregiver training
- Respite care
- Limited supplemental services, such as transportation and home modifications.
