Long Term Home Health Care Program

Government program assists individuals with disabilities and older adults, medically eligible for placement into a nursing home, remain in their own homes by providing a coordinated plan for medical, nursing and rehabilitative care. Services include:
– Medical services
– Care management by registered nurse
– Home delivered or congregate meals
– Home modifications
– Respiratory therapy
– Respite
– Adult day care
– Transportation
