Programs of All-Inclusive Care for the Elderly (PACE) – Colorado

PACE programs in Colorado coordinate and provide the entire continuum of care and services to their older enrollees with chronic care needs to enable them to continue living safely in the community as long as possible. Care and services include:
– Adult day health care
– Medical care
– Home health care and personal care
– All necessary prescription drugs
– Social services
– Respite care
– Hospital and nursing home care when necessary
