Community Long Term Care Program (CLTC)

Community Long Term Care (CLTC) offers programs to help individuals who want to live at home, need assistance with their care, and are financially eligible for Medicaid.

Services may include care management, in-home care, environmental modification, home delivered meals, adult day health care, respite care, incontinence supplies, limited medical equipment.

Under the Community Choices waiver, certain family members, excluding spouses and legal guardians, can be hired and paid as the personal care provider.
