Elderly and Disabled Waiver

Government program assists adults who require nursing level of care remain in their own homes by helping to pay for home and community based services including:
– Care management
– In-home care
– Adult day health
– Respite
– Transportation
– Home-delivered meals
Also, assists residents of nursing homes return to the community, if they are able to do so safely, by managing the transition and providing the appropriate community based services.
