Family Care

Government program assists low income individuals who need help with activities of daily living by providing long term care and home and community based services. Services include:
– Care management
– In-home care
– Adult day services
– Home-delivered meals
– Home modifications
– Skilled nursing
– Mental health services
– Occupational, physical and speech therapy
– Help coordinating primary health care
– Independent living skills training
– Vocational rehabilitation
– Supplemental services
Program is only available in Columbia, Fond du Lac, Kenosha, La Crosse, Milwaukee (serves the elderly population only), Ozaukee, Portage, Racine, Richland and Sheboygan Counties.
