Support for People Living with Dementia & Their Caregivers
Caregiving for a family member with dementia can be an emotional rollercoaster. The challenges can feel isolating, and the journey can seem endless. But there’s hope.
In partnership with Tembo Health, FCA is here to help. Through the new innovative GUIDE program, we’re offering comprehensive dementia care that addresses the medical needs of individuals and their families as well as help with support services.
What is GUIDE?
GUIDE (Guiding an Improved Dementia Experience) is a new Medicare program that allows FCA and Tembo Health, alongside your Primary Care Provider, to provide added support and resources for people and families living with dementia at no cost to you.

Care Navigator
Your personal coach to help with care planning and support.
24/7 Helpline
Call anytime for urgent needs or questions.
Caregiver Support
Education, training, and support groups.
Respite Care
Up to $2,500 per year for in- home aides or adult day programs.
Community Connections
Help with transportation, meals, and local resources.
How Does It Work?
- Reach out to us to schedule a call with one of our experts.
- Schedule a Tembo Medicare evaluation and get diagnosed if necessary.
- Complete application surveys.
- Receive notification of acceptance into the program.
- Have regular check-ins with a care navigator or other dementia specialists.
- See your PCP as usual for your health care.
- Reach out 24/7 to Tembo’s virtual urgent care for emergencies.
Who can join?
You may be eligible based on the person you care for if they:
- Have a diagnosed dementia condition
- Have Traditional Medicare (Parts A & B) as your primary insurance
- Cannot be on a Medicare Advantage Plan
- Live at home or in a community setting
- Not long-term nursing care
- Not enrolled in hospice or PACE
- Cannot be on a Medicare Advantage Plan
- Not long-term nursing care
