‘Just in Time’ Hospital Tip Sheets for Family Caregivers
Our Just-in-Time tip sheets for caregivers offer helpful advice on how to communicate with hospital staff when the person you’re caring for needs surgery, is hospitalized or ends up in the Emergency Department.
The tip sheets include what questions to ask and what information caregivers need to have ready. Remember, caregivers play a crucial role in the care of someone with Alzheimer’s disease-related dementias, especially during transitions in care (moving from one care setting to another, like hospital to home or nursing home).
By working together, we can improve the quality of care for these individuals.
